3.本項(xiàng)目的特定資格要求：

合同包1(LED公告公示屏采購(gòu)項(xiàng)目)特定資格要求如下:

（1）供應(yīng)商具有獨(dú)立承擔(dān)民事責(zé)任能力的法人、其他組織或自然人，并出具合法有效的營(yíng)業(yè)執(zhí)照或事業(yè)單位法人證書(shū)等國(guó)家規(guī)定的相關(guān)證明，自然人參與的提供其身份證明書(shū)；（2）供應(yīng)商提供2025年度完整的財(cái)務(wù)審計(jì)報(bào)告（至少包括資產(chǎn)負(fù)債表、利潤(rùn)表、現(xiàn)金流量表及財(cái)務(wù)報(bào)表附注，成立時(shí)間至提交投標(biāo)文件截止時(shí)間不足一年的可提供成立后任意時(shí)段的資產(chǎn)負(fù)債表），或開(kāi)標(biāo)前半年內(nèi)其基本存款賬戶開(kāi)戶銀行出具的資信證明；（3）供應(yīng)商提供2025年9月1日至今至少六個(gè)月的納稅證明或完稅證明，依法免稅的單位應(yīng)提供相關(guān)證明材料；（4）供應(yīng)商提供2025年9月1日至今至少六個(gè)月的社會(huì)保障資金繳存單據(jù)或社保機(jī)構(gòu)開(kāi)具的社會(huì)保險(xiǎn)參保繳費(fèi)情況證明，依法不需要繳納社會(huì)保障資金的單位應(yīng)提供相關(guān)證明材料；（5）供應(yīng)商提供具有履行本合同所必需的設(shè)備和專業(yè)技術(shù)能力的聲明；（6）供應(yīng)商出具參加政府采購(gòu)活動(dòng)前3年內(nèi)在經(jīng)營(yíng)活動(dòng)中沒(méi)有重大違法記錄的書(shū)面聲明；（7）供應(yīng)商須提供法定代表人授權(quán)書(shū)（法定代表人或負(fù)責(zé)人直接參與談判，須提交法定代表人證明書(shū)）；（8）供應(yīng)商應(yīng)在信用中國(guó)（www.creditchina.gov.cn）未被列入重大稅收違法失信主體、在中國(guó)執(zhí)行信息公開(kāi)網(wǎng)（http://zxgk.court.gov.cn/）中未被列入失信被執(zhí)行人及在中國(guó)政府采購(gòu)網(wǎng)（www.ccgp.gov.cn）未被列入政府采購(gòu)嚴(yán)重違法失信行為記錄名單（處罰期限屆滿的除外，如相關(guān)失信記錄已失效，供應(yīng)商需提供相關(guān)證明資料）；（9）單位負(fù)責(zé)人為同一人或者存在直接控股、管理關(guān)系的不同供應(yīng)商不得參加同一合同項(xiàng)下的政府采購(gòu)活動(dòng)。
