（1）具有獨(dú)立承擔(dān)民事責(zé)任能力的法人、其他組織或自然人，并出具合法有效的營(yíng)業(yè)執(zhí)照（附2025年度報(bào)告）或事業(yè)單位法人證書等國(guó)家規(guī)定的相關(guān)證明，自然人參與的提供其身份證明；（2）法定代表人授權(quán)書（附法定代表人身份證復(fù)印件）及被授權(quán)人身份證（法定代表人直接參加只須提供法定代表人身份證）；（3）具有安全生產(chǎn)許可證書、公路工程施工承包三級(jí)或以上資質(zhì)證書、擬派項(xiàng)目經(jīng)理具有公路工程專業(yè)二級(jí)建造師或以上資質(zhì)證書和安全B證，且無在建工程證明；（4）財(cái)務(wù)狀況報(bào)告：提供2025年度經(jīng)審計(jì)的財(cái)務(wù)報(bào)告；或開標(biāo)時(shí)間前三個(gè)月內(nèi)銀行出具的資信證明及基本存款賬戶開戶信息；（5）稅收繳納證明：提供繳費(fèi)所屬日期為投標(biāo)截止時(shí)間6個(gè)月內(nèi)任三個(gè)月（投標(biāo)截止時(shí)間當(dāng)月不計(jì)入）的增值稅（或企業(yè)所得稅）繳費(fèi)憑據(jù)或稅務(wù)機(jī)關(guān)出具的完稅證明/在法規(guī)范圍內(nèi)不需提供的應(yīng)出具書面說明和證明文件；（6）社會(huì)保障資金繳納證明：提供截止至開標(biāo)時(shí)間前六個(gè)月任意一個(gè)月份的社會(huì)保障資金繳存單據(jù)或社保機(jī)構(gòu)開具的社會(huì)保險(xiǎn)參保繳費(fèi)情況證明，依法不需要繳納社會(huì)保障資金的單位應(yīng)提供相關(guān)證明材料；（7）參加本次政府采購活動(dòng)前三年內(nèi)在活動(dòng)經(jīng)營(yíng)中無重大違法記錄的書面聲明；（8）須通過“信用中國(guó)”網(wǎng)站（www.creditchina.gov.cn）和中國(guó)政府采購網(wǎng)（www.ccgp.gov.cn）查詢相關(guān)主體信用記錄（查詢?nèi)掌跒閺恼袠?biāo)文件發(fā)售之日起至投標(biāo)截止日前），以網(wǎng)頁截圖加蓋投標(biāo)人公章為準(zhǔn)，對(duì)列入失信被執(zhí)行人、重大稅收違法案件當(dāng)事人名單、政府采購嚴(yán)重違法失信行為記錄名單及其他不符合《中華人民共和國(guó)政府采購法》第二十二條規(guī)定條件的將拒絕其參與政府采購活動(dòng)。供應(yīng)商必須為陜西省政府采購網(wǎng)在冊(cè)供應(yīng)商；（9）本項(xiàng)目為專門面向中小企業(yè)，采購項(xiàng)目須根據(jù)《政府采購促進(jìn)中小企業(yè)發(fā)展管理辦法》（財(cái)庫﹝2025﹞46號(hào)）的規(guī)定提供《中小企業(yè)聲明函》；（10）單位負(fù)責(zé)人為同一人或者存在控股、管理關(guān)系的不同供應(yīng)商，不得參加同一合同項(xiàng)下的政府采購活動(dòng)；（11）本項(xiàng)目不接受聯(lián)合體投標(biāo)。
