3.本項(xiàng)目的特定資格要求：

合同包1(門診樓安全隱患消除修繕項(xiàng)目)特定資格要求如下:

（1）申請(qǐng)人應(yīng)為向采購(gòu)人提供工程及相應(yīng)服務(wù)的法人或其他組織；（2）截止至申請(qǐng)文件遞交截止時(shí)間之前，申請(qǐng)人未在“信用中國(guó)”網(wǎng)站（www.creditchina.gov.cn）中被列入“信用服務(wù)”查詢欄目中嚴(yán)重失信主體名單以及重點(diǎn)領(lǐng)域嚴(yán)重失信主體名單；未被列入“中國(guó)政府采購(gòu)網(wǎng)(www.ccgp.gov.cn)”政府采購(gòu)嚴(yán)重違法失信行為記錄名單，不得被列入“中國(guó)執(zhí)行信息公開網(wǎng)”失信被執(zhí)行人名單，申請(qǐng)人基本信息及申請(qǐng)人擬派項(xiàng)目經(jīng)理基本信息應(yīng)在“陜西省住房和城鄉(xiāng)建設(shè)廳”可查詢；（3）申請(qǐng)人參加本次招標(biāo)活動(dòng)前三年內(nèi)，在經(jīng)營(yíng)活動(dòng)中沒(méi)有重大違法記錄；（4）單位負(fù)責(zé)人為同一人或者存在直接控股、管理關(guān)系的不同申請(qǐng)人，不得參加同一合同項(xiàng)下的招標(biāo)活動(dòng)；（5）申請(qǐng)人須具備建設(shè)主管部門頒發(fā)的房屋建筑工程施工總承包三級(jí)（含）以上資質(zhì)，且具備有效的安全生產(chǎn)許可證，并在人員、設(shè)備、資金等方面具備相應(yīng)施工能力且無(wú)不良記錄；（6）申請(qǐng)人擬派項(xiàng)目經(jīng)理應(yīng)具備建筑工程專業(yè)二級(jí)（含）以上注冊(cè)建造師執(zhí)業(yè)資格及有效的安全生產(chǎn)考核合格證，在本單位注冊(cè)且未擔(dān)任其他在建工程項(xiàng)目的項(xiàng)目經(jīng)理；（7）本項(xiàng)目不接受聯(lián)合體投標(biāo)。

3.獲取資格預(yù)審文件時(shí)攜帶單位介紹信及經(jīng)辦人身份證復(fù)印件加蓋公章，資格預(yù)審文件每份300元（售后不退，謝絕郵寄）；
